Health, according to the dictionaries, is a feeling of well-being. That was the old definition! The modem definition of health says that you must not only feel all right but also that you must not be aware of any pain or irritation anywhere in your body. Moreover, you must have what the quacks used to call vim, vigor and vitality-a feeling of positive health. You must have the desire to be up and doing. You must be free from fatigue. You must not get tired easily.
Once we thought that we could give people a lot of simple rules for keeping their bodies in health. Now we have learned so much more about the nature of disease that the rules have become complicated. Doctors used to say that you could take three or four meals a day and that this would take care of your needs as far as health was concerned. The list included breakfast at 8 o’clock, dinner or lunch at noon, perhaps a supplement in the form of tea or a little snack around 4: 30 in the afternoon, and then supper or dinner about 7: 30. Nowadays we know that some people do even better on six small meals a day; others seem to be quite healthful with just a light lunch but with a fairly good breakfast and not too large a dinner. We are so individual in our construction and in our constitutions that it is impossible to put down a rule for positive patterns of eating or amounts of eating that will apply to all people.
The same considerations apply to rest and sleep. Some people need more rest and more sleep than do others. Some are able to get along quite satisfactorily on six hours; others seem to require as much as ten or twelve hours of sleep to be at their best during the waking hours. Eight hours of sleep might be put down as an average, with recognition, however, that some are above and others below the average.
The hygienists used to say that everyone ought to have at least a quart of water a day, recommending eight glasses-one before breakfast, one between each meal, one at each meal and one before going to bed. Nowadays we know that our appetites and our sense of thirst may be a reasonable indication of what we ought to have in the way of fluids. One health expert recommended three pints of water daily, indicating, however, that it could be taken in tea, coffee, cocoa, milk, soup or some other liquid. Thus the day of rest varies among different races of people and among various religions. Similarly attitudes toward food vary with various races and religions. The diet should be free from infectious material and visible dirt. The point of view toward life must be moderate and temperate.
In the Middle Ages the School of Salerno developed a book called “The Regimen of the School of Salerno”. The chief emphasis in that guide to health and hygiene was moderation in all things. Oxygen In The Body
The tissues of the body have to have oxygen to live. When the supply of oxygen is inadequate the condition is called “anoxia.” The shortage of oxygen may be apparent in the circulating blood. The red cells of the blood may be inadequate in amount or in the red coloring matter necessary to carry oxygen. Anything that blocks the circulation will also block the oxygen supply. Sometimes the cells of the body are unable, because of changes, to take up the oxygen that reaches them.
Since all parts of the body must have oxygen, a shortage will affect all of them. However, some tissues of the body are much more dependent on oxygen than are others. Most sensitive of all are the tissues of the nervous system. Sudden lack of oxygen to the nervous system results in impairment of judgment, lack of co-ordination of movements, and a condition which, in general, resembles that of a person who is drunk. After the lack of oxygen has persisted the person becomes fatigued, drowsy, inattentive, and unable to respond to ordinary stimuli.
If lack of oxygen to the brain persists, death will result from inability to breathe. A failure of sufficient oxygen to reach the liver and the muscles where foods are broken down to their ultimate condition for use by the body, results in acidosis and is therefore also incompatible with life. The increase in red blood cells and hemoglobin in response to anoxia may begin gradually and continue for weeks. At high altitudes the total increase may reach 40 per cent above the usual.
Cyanosis And Argyria
The word “cyanosis” means blueness, but in medicine it is restricted to the kind of blueness that follows a reduction in the amount of hemoglobin or red coloring matter in the blood. A condition called “argyria” which is due to deposit of silver in the skin gives a silvery-blue appearance. Blueness due to lack of oxygen in the blood is best seen in the lips, the white of the eye, the fingernail beds, the ears and the area over the cheek bones.
Certain poisonous substances including drugs may lead to cyanosis, by changing the nature of the hemoglobin or red coloring matter of the blood. Among these drugs are the nitrates which are sometimes used to dilate blood vessels and lower blood pressure. Also hydrogen sulfide and acetanilide may have this effect. When people are poisoned by carbon monoxide gas, the blood develops a cherry-red color rather than blue. Occasionally people who take sulfonamide drugs get bluish blood due to a chemical change.
When there is any interference with the flow of blood through the skin the color may seem blue or a pale bluish-gray. Such difficulty may come from a weak heart, an obstruction of the flow of blood or simple exposure of the skin to severe cold. Some people suffer constantly with cold and bluish hands and feet because of poor circulation in the extremities.
The doctor must determine whether the difficulty is due to the heart, or the lungs or some trouble in the blood itself. By special signs such as clubbing of the fingers, the duration of the condition in relationship to employment, examinations of the heart and lungs, and chemical and physical studies of the blood, he can make the distinction.
Too Many Red Cells-Polycythemia
Doctors put together names of diseases frequently out of portions of words. “Poly” means multiple or too many; “cyth” refers to cells; “emia” means the blood. An alarming increase in the number of red blood cells might also be called an erythrocytosis, which merely means a condition of the red blood cells. An excess number of the cells may develop as a result of an insufficient oxygen supply, such as occurs at high altitudes, or as a result of an excess manufacture, producing a disease the cause of which is not known. This true polycythemia is also called Osler’s disease and Vaquez’s disease after physicians who first noticed it.
Polycythemia comes on gradually and persists for ten or twenty years. The person with this condition has constantly a deep red flush which may have a bluish appearance. Usually the spleen and the liver are enlarged. The blood clots easily. Hemorrhages in various parts of the body are not uncommon. Whereas the blood count ordinarily is around five to six million, the count rises in this condition to nine to twelve million cells in each cubic millimeter of blood. Among unusual causes of secondary polycythemia, in addition to residence at high altitude, are disturbances such as silicosis, which interfere with receipt of oxygen by the lungs; abnormalities of circulation of the blood through the lungs; cases which occur in infants that have been unable to get a good oxygen supply before birth; and even certain tumors of the brain and failure of the adrenal glands. Among methods of treatment now used to control excess production of the cells are X-ray of the bone marrow, giving of radioactive phosphorus and use of the nitrogen mustards.

Worry is a protracted or recurrent act of the mind, which always fails to result in a constructive solution of the question and usually ends in confusion, fatigue, and emotional instability. Even when the conclusion happens to be contrary to your wishes, it may be accepted as a conclusion and thus worry can be avoided. Such an effort is constructive, whereas worry is always destructive.
The ordinary dictionaries describe worry as feeling or expressing a great deal of care and anxiety, manifesting unrest or pain, fretting, chafing, being anxious or fearful. Since all of these reactions are undesirable from the point of view of their effects on the body, worry is a most undesirable characteristic.
Many people insist that they never worry. These are the people who have learned to reason themselves out of anxiety over situations in which they find themselves. The process is known as “rationalizing.” Other people developmental tranquility or peace of mind by accepting a belief which eliminates from consideration anything displeasing to them. Such a process is not rationalizing, but may achieve the same effect if the person can shut out completely any problem that disturbs him. Most people find peace of mind necessary if they are to accomplish their responsibilities in the business world or in the home. If one does not have such peace of mind there is a constant feeling of insecurity, a constant fear of a threat to life itself or to the life situation of the person concerned. As a result, energy is squandered and the reserve of the nervous system is exhausted, so that the person becomes tired, worn, distressed and may have what is commonly called a nervous breakdown.
When worry appears to this extent, the effects manifest themselves on different portions of the body. If the worry is related to the heart the person feels palpitations, extra and light beats of the heart, and similar manifestations; such a person may focus attention unduly on the pulse or the blood pressure or some other factor related to the circulation. If the nervous condition brings the focus of attention on the stomach and ~ bowels there may be constipation, diarrhea, or other manifestations even more serious.
Many a person endeavors to escape from worry by fleeing into an addiction to drink, to drugs, to sedatives, to gambling, or to other practices that are known to be against the best interests of humanity. The escape is only temporary, and the trouble returns just as soon as the liquor or the drugs have worn off. There is no doubt that a restful night’s sleep, a vacation, indulgence in outdoor sports, or even the theater or the movies may be utilized to better advantage as means of
escape from the reality of worry.
Emotional Disturbances
Most people have minor emotional disturbances that are not recognized but, because of their effects on the general health, have medical significance. Many people in industry, in labor, and in public life get along, although they suffer at all times with difficulties of adjustment to their environment. An analysis of the people who come to doctors’ offices .revealed that from 30 to 60 per cent come as patients primarily because of complaints due to emotional disorders, which are reflected as physical disorders. Treatment is not fully satisfactory unless it takes care of the mental as well as the physical factors.
Our population has become an aging population. People live much longer than was common fifty years ago. As people get older they tend to develop more emotional experiences, and this has greatly multiplied the need of psychiatric help. According to the available figures one out of every twenty people will need advice or guidance from an expert for severe emotional illness at some time during his lifetime. This is indicated by the fact that more than one half of all the veterans in veterans’ hospitals are psychiatric patients. Thirty-seven per cent of releases from the army for medical reasons were for psychiatric disorders. A total of 51 per cent of all medical separations from the military service were due to personality disturbances. In addition, people who are mentally disturbed constitute about one-half of all of the patients in hospitals in the United States.
While modem medicine has much knowledge of psychiatric conditions, far more still remains unknown than is actually known. Methods of treatment developed in recent years include the successful use of the tranquilizing drugs (see below), electric shock, psychoanalysis, the use of occupational and recreational therapy. Since the number of competent experts capable of treating such patients is insufficient, study is now being made of the treatment of mentally ill patients in groups. This is called group psychotherapy.

What a businessman wants is a worker who can respond to the particular problems and procedures of the job for which he is employed. The boss seldom wants to be troubled about the general personality of an employee or the question of how he gets along with his wife. Nevertheless, that very situation may be important in relation to the quality or amount of work. Problems may arise which are due to it neurosis or psychosis in some employee whose mental condition has not been recognized.
Mental hygienists are convinced that training ought to be made available to employment managers or to the workers in the personnel divisions of industries. Workers are frequently transferred into personnel departments because they appear to be able to get along well with other people -but sometimes because they are hard and skeptical. Generally they work out their own techniques, whether for the handling of personal problems or for the selection of new employees.
Already there are plenty of reports of instances in which employees who failed to respond acceptably to their executives were given scientific study and thus saved for the organization. These children will be brought into social and economic competition, and the contrast with normal children, coupled with pressure from forces behind them at home and even from their association with competitors, may result in a reaction and in the formation of attitudes which lead to permanent disturbances. There are many causes of mental breakdown and many classifications of mental disturbance. Research has been intensified on dementia praecox, in which the so-called insulin shock, metrazol shock, and electric shock treatments are being tried. Modem medicine also offers new forms of study, including analysis of the mental processes, leading to recognition of the underlying factors in mental disturbances.
The wise man need not fear such a catastrophe. He should know that scientific methods of diagnosis and treatment are now available.
